Medicare Notice
White Olive Direct Personalized Care memberships include services not covered by Medicare, such as metabolic optimization, hormone therapy, advanced diagnostic testing, and longevity evaluations. Medicare will continue to be billed for all Medicare-covered services. Membership is optional and does not replace Medicare benefits.

To rejoin after discontinuing membership previously, $150 re-enrollment fee.

If the monthly membership payment is not received on the due date, there will be a 30-day grace period to pay membership fees in full. Failure to do so will result in automatic cancellation of membership.
